Medical assessment without ESA50

I made my initial claim for ESA on 14 January this year, I have been given an appointment on 13 May for a face-to-face assessment but have not yet been asked to fill in an ESA50. Is this right? Everything I've read seems to indicate that I should have been asked to fill in the form, but I haven't been sent anything other than the appointment...

Welcome to the forum.

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

All claimants should be sent an ESA50 and Maximus, the assessment provider, have assured us that this will happen.

I would start by phoning Maximus, their number will be on the appointment letter and ask for an ESA50 to be issued and the appointment to be deferred at no fault to you, until you have had time to complete and return it. See the following

If they refuse then your only option it download the ESA50 and complete it before the assessment, you can then hand it in on the day. See

I would also suggest that you send a copy to the DWP, marked for the attention of the Decision Maker.

Thanks. I've just spoken to the health assessment advisory service and they referred me to DWP - it seems that they sent the forms to an address I moved out of 13 years ago.... Despite my telling them about this error when they stopped my payments due to some post being returned as undeliverable! You'd think they would have resent the forms at this point, but I suppose that that level of common sense might be asking a bit too much! I'll download one now and take a look at what's in store, unfortunately I don't have a printer at the moment so they are posting me a form to fill in. They have told me I don't need to send it to the DWP, just to take it to my assessment. The assessment people have agreed to defer the appointment if I'm struggling to fill the form in on time - so could have been worse!
