Hello,
I think I wrote a similar post a while ago. Basically I have been on assessment rate ESA since Oct/Nov 2013 (backdated 3 months from that date). I have not in this time been sent an ESA-50 and have not contacted the DWP about this because I didn't want to go through the unnecessary stress that a medical assessment would involve.
Is it important that I contact the DWP/send an ESA-50 form? what will happen with my application if I don't send a ESA-50? Am I likely to be called for assessment without a medical?

It's entirely up to you whether you contact the DWP or not.
The ESA50 is you opportunity to explain your limitations in your own words without constraint, if you do not complete one then the Decision Maker can only make a Decision on the information provided by people who at best may only know your problems from a distance and at worst are evaluating you over a one hour interview, never having seen you before.
I'm not sure what you mean by
The ESA assessment is not a medical assessment of your conditions but rather a functional assessment of your capabilities in relation to the ESA Descriptors, a very different thing.
