MEDICAL IN 3 WEEKS was created by peter
The nerves reappear once again. I was awarded nil points back in May 2012 went on to win my appeal in September 2013 with 15 points in one descriptor.
panel said no review for at least 12 months so I know I've bee lucky to last this long before review.
my question is that my diverticulitis is getting worse with time and im still under the consultant whom I see 2 weeks after this medical and he wants to do some big op removing part of my colon.
what evidence do I need to take with me on the day?

Have you received, completed and returned an ESA50?
If you have not then you should contact Maximus, there should be a phone number on the appointment letter, to ask for one to be sent to you and for the appointment to be delayed until you have had a chance to complete and return it.

Have you received, completed and returned an ESA50?

Thinking back I may have filled in one of them back in September and have heard nothing since.
will this be the form they will work off?

If you completed an ESA50 last year then this will be the form that they will work from.
If you condition has changed since then, then you can provide additional up to and including the day of the assessment. If you do then try and keep it in the same format as the ESA50 and remember that any limitations that you describe really need to be related to the ESA Descriptors to have any effect. You might also want to send a copy to the DWP office dealing with your claim, mark it for the attention of the Decision Maker.
Finally, take a copy with you to the appointment and ask the assessor to read it and include it in your file before they start the interview.
