Hi ive been getting low rate DLA care and mobility for 6 years and its due to finish soon. Ive been sent a DLA form for looking again at my claim and am confused as im in an area that has PIP. I didnt have a phone interview and am stressing that I may put the wrong information down as im claiming for PIP. I had a medical for ESA earlier this year and have been put in the support group. Please could someone tell me the best way to fill in the DLA form when it will be for PIP and also will they take my medical for ESA into account? Thanks in advance.

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

If you have been sent DLA forms then you are being re-assessed for DLA and not PIP.

Although your area is now live for PIP, if this did not happen more than 20 weeks in advance of your current DLA ending, then you will not have been invited to claim PIP.
