I am getting nowhere fast and hitting a brick wall with Atos and DWP. We have possesion of copy Atos HCP's face to face report.This is the form they send from the consultation to DWP decision maker. The reports are 28 pages.We have NO page 4. We would like to know what the HCP fills in on Page 4. Page 3 is about Medication and social and occupational history. Page 5 is about Functional history. If anyone has a copy of their own report would really appreciate if they could have a look and let me know what subject HCP writes about on Page 4. Thanks. PS worth checking all page numbers when receiving copies of these reports to make sure all there. They don't seem to want to send me our page 4 for some reason.
Hi Bigcat, I just got mine today at 3rd time of asking. My page 4 is just a blank "Continuation" box. I assume this is to add further info/comments from page 3 if needed. Hope this helps.
