1. There is no online version of the PIP1 form, it can either be competed over the phone when you make a claim or you can request a paper copy is sent to you and complete it your self.

2, It only refers to your previous DLA information but this does not stop the DWP or the Assessment Provider from using information from your ESA claim, however, it is unusual for them to do this so you might want to include this information with your PIP 2 form, whilst there are some similarities between the Descriptors there are also significant differences so you might want to draw the attention of the assessor and the Decision Maker to those areas that you believe are relevant.

3. Yes the PIP2 form is the "how your disability affects you" form. The PIP2 is not available for download, it is stamped with a bar code unique to the claimant and must be used to document your limitations, however, you can attach additional sheets to it as necessary if you are unable to fit your responses in the comments box.

It's been 20 years since we had to fill in a proper DLA form for my husband. He currently receives the same DLA (HRM & MRC ) he first received back then with the only chane being 'Life' changed to ' Indefinite'. Since then,have had a ' right Payment programme' form and 2 ESA claims which we past with flying colours and support group awad. However,it's only the last 2 years since they finally diagnosed hubby with Alzheimer's and this is now effecting him along with his long standing chronic arthritis from his original claim. We never reported a 'Change of Circumstances' because his care part of his original DLA claim only changed slowly with him now needing round the clock care both day and night. Will the DWP take this into consideration when they ask on any would be PIP form if we want them to apply all the details from hubby's last ( 1995) DLA claim ?

I've moved your post to it's own topic as it is not fair to the original poster to confuse them with answers not relevant to their situation.

Even though the last full review of your husbands DLA claim appears to have been some time ago, the fact that it resulted in an indefinite award may still make it worth including with regard to PIP. However. the criteria for PIP are significantly different from DLA and you will not be able to rely on the fact that there is a current DLA award.

One difference that would appear to be relevant is that PIP does not specifically look at the times during the day that a claimant is affect, as DLA did, but rather the severity of the claimants problems, so needing day and night time care will not directly lead to points, also PIP is very specific in the areas that it looks at..

So you will need to explain detail and in reference to the PIP Descriptors how your husbands day to day activities are limited by his conditions. See the PIP Claim guide
