Hi,

I sent my form for sdp around two weeks ago they havent received the forms yet. Do you think they have been lost by now and to send another form?

They dont seem very helpful on phone and havent called me back regarding this. Also i was awarded pip in august should this be backdated?

I would have expected your form to have made it to the relevant department by now, despite the vagaries of the DWP internal mail system, so it may well be worth your sending it again.

Yes, it will be backdated to when you became eligible.
