Hi Fg1,

Welcome to Benefits and Work :)

The PIP 1 Form is to initiate your PIP claim process and captures your personal details and basic information about your medical conditions and resulting limitations.

The PIP 2 Form (aka The How your Disability Affects you Questionnaire) is used as part of your assessment to see whether you score enough points under the PIP Activities to warrant an award of PIP Daily Living and /or Mobility.

It is in the PIP 2 that you should go into detail regarding your limitations/disabilities, and you should provide any supporting medical evidence that you have with the PIP 2.

You can download samples of The PIP 1 and PIP 2 from the links in our :

PIP Claims Guide. (2nd Link down the page)
