Could someone help me please as I'm having difficulty finding a copy of the PIP2 ( how your disability effects you) form so I can sit down tonight and familerize myself with it.
Sorry to be a pain,hubby has multiple issues including Alzheimer's and its worn me out today.

There is a sample of the PIP2 form on the following page. You must not use this for any claim that you make, the form that you will be sent as part of a claim has a unique bar code on it and must be the one returned.
