www.gov.uk/pip/how-to-claim Please be aware that one of our members has had problems doing this, so If you can get someone to phone on your behalf then that would be better.
If you do write then you need to make sure that you keep track of the timescales and escalate the problem if you are approaching the date when claim needs to be made by.
