Hi H,
The initial phone call is to initiate your transfer over to PIP, and they will fill in the basic PIP 1 Form with you over the phone.
You should then receive copy of the filled in PIP 1 Form through the post for you to read through, sign and return if you agree with the contents.
Once you have signed and returned The PIP 1 Form, you should then receive The PIP 2 "How your Disability Affects You" Questionnaire through the post to fill in and return.
It is the PIP 2 where you should go into detail about your medical conditions and resulting limitations, and it is with this form that you should attach any supporting letters or medical evidence.
As long as you abide by any deadlines given to return forms etc, within the transfer process, your current level of DLA payments will continue until 4 weeks after the date of the new PIP decision, no matter what that PIP decision may be.

PIP Self Test
If you do not think that you will be able to fill in The PIP 1 form over the phone, you can request that they post one out to you when you ring up.
Again, it would be important that you abided by any deadline dates given.
If when you receive the PIP 2 Form you are struggling to fill it in and return it by the deadline, you can request a two week extension.

Regarding the HCP who carries out the f2f if needed, they probably won't take any notice of your request. It'll be whoever is on that day.
For that reason it would be reasonable, I think, to include paperwork about your condition you have found on the web, especially if you can link it to the answers you give on the PIP2. ie this paper explains x which contributes to my difficulties in carrying out descriptor y in a timely manner. etc.
