I have filled in pip2 form and had my assesment but to my recollection I've not received a pip 1 form and if this is the one that's filled out on the telephone surely I should have received it to clarify it and sign and send back . I haven't got it. Should I be worried ?

The PIP1 is an administrative form that captures basic information about the claimant so that a PIP claim can be created on the DWP computer system, it would have been completed on your behalf when you made the initial phone call to start your claim for PIP.

They used to send it out for signature but now don't, there's nothing to worry about, if there was an issue then you wouldn't have received your PIP2 form.
