I am considering moving in with my friend. Will changing my address trigger a reassessment for my ESA/DLA. My mum gets carers allowence for me and I'd be moving out of her house. My mum may decide to give up caring for me in my new home.

There's no reason that a change of address will cause a re-assessment of either your ESA or DLA.
Obviously you will need to inform both departments, you cannot rely on one informing the other.
If your ESA is Income Related in part or in whole, it is important that you explain your new living arrangements in detail to the DWP, they have a habit of assuming that people in this situation are living as partners. Separate rent books would be best, but at the very least you may need to show that you live separately from each other. If you are not receiving ESA(IR) then your living arrangements will not be a problem.
If your mother stops caring for you then you both should contact the Carers Unit. See
