ESA50 help

im up all night stressing because this form needs to be in asap and im struggling to find Q11-18 on EA50 about mental health, then this is all i can find and it tells me its not the latest so now i don't know what to do.

''You can download sample pages from the Guide to Employment and Support Allowance claims on mental health and learning difficulties grounds (Full guide 86, pages A4, June 2012 - this is not the latest version of the guide).''
