it just says ESA04/15, When I got a copy of y report from the last face to face the examiner did write, unlikely to improve

I haven't heard of an ESA04 before and I can't find an example.

Does it bear sufficient resemblance to the ESA50 for you to use the ESA Claim guides to complete it?

If not, can you scan it and send a copy to the Office, they may be able to help.

Hi Z,

Are you sure that it is not an ESA50A 04/15, the 04/15 (April 2015) will simply be the date of the version of the form.

An ESA50A may be issued where the Decision Maker (DM) has identified a claimant as having Limited Capability for Work (LCW) (WRAG qualification) based on medical evidence, the claimant may be sent an ESA50A to decide whether they also have Limited Capability for Work-Related Activity. (LCWRA) (Support Group (SG) qualification)

The ESA50A and any other medical evidence will be reviewed by a Maximus ESA Assessor, If a recommendation on whether the claimant does, or does not, meet the criteria for inclusion in the support group cannot be made on the basis of the evidence.

So basically, an ESA50A is a shortened version of the ESA50.

If you still have any doubts, compare the questions in the Form that you received through the post, with the questions in The PDF ESA50 that you have downloaded.

it does say ESA50 04/15. its 20 pages long and the letter with it just says, capability for work questionnaire. its white and pink. when I filled out the pip form, you sent me a link to the exact legally acceptable/suggested responses, question by question. did you send one for this ESA50?

Hi Z,

That is the normal ESA50 then, ( ESA50 version 04/15 {version April 2015} ) and you can use The PDF version which is exactly the same.

There is a download link for The PDF ESA50 in the FAQ : Using The PDF Version Of The ESA50 Questionnaire

Yes, there is also a link that takes you to our ESA Claims Guides which you should refer to whilst filling in The ESA50.

thanks, im now at the mental health part which is important for my case also and its just skipped and told me to go search the other area and im struggling to navigate the site. Question 11-18 i believe, could someone please help?
