ESA50 problem

I have just received my Capability for Work ESA50 this morning and my stomach is churning already. I have had my health problem for many, many years and have filled these forms in before, with the help of this website for the last 2. I have been for one face to face interview which was abandoned because I got so stressed and was clearly unwell and in discomfort.
My problem is that I have lack of "evidence" to back up my claim now.
I have recently changed doctors so no one knows me or my situation at the new health centre. I do not seek medical help for my illness/incapacity now because I have literally seen every department at my two local hospitals over the last 15 years. They were unable to help me long term so I receive no treatments or follow up appts now.
I see a private osteopath, who I have been with since I was forced to give up work in 1999. He keeps me going, along with another "hands on" therapist who also makes a difference and keeps me ticking over too, so that I can function.
I have a copy of a letter from 3 years ago from my old Gp stating how I am incapacitated but that's it. Nothing has changed for me since the last face to face assessment.
Any suggestions as to how I approach this?

Hi Amelia.
Sorry you are getting so distressed; it's difficult to know where to start when you are feeling so upset.
First of all, I would make an appointment to see your new GP and ask him/her if he has access to your records from your old GP. I think records have to be passed from GPs in order that they are aware of past medical history. If the old records are available you can ask for copies of records that support your claim. You may have to pay for these records.
Speak with your new GP, explaining to him why you have stopped asking for medical help. If he doesn't know your history then tell him, and ask him if he is prepared to write you a letter outlining your medical condition and how it is likely to affect you. Show him the letter from your last GP
Your osteopath will, I am sure, be happy to provide you with a supporting letter, outlining the treatments he has been giving you, as will your other therapist.
Fill in the form with the help of our guides, focusing on how your medical condition is damaging your mental health.
Try these suggestions and see what you can come up with. If we can help any further, please get in contact again. I hope all goes well for you.

Thank you for your reply. I actually have copies of all hospital reports thanks to my old GP/employer of many years. My biggest worry is that they are dated 2013 back to 1993 - nothing more recent. The thought of having to explain my complex problem and history to a new doctor is daunting but I guess I will have to bite the bullet!
