hello , i was sent an esa50 04/15 form arrived christmas eve , a bit worried my husband filled it in for me , and i got supporting letters from Gp And psychiatrist stating my schizoaffective disorder and my ptsd and why in there opionion i cant work , the only thing i didnt get was an up to date care plan from my cpn so i had to send the last one i had which was 2014 , i know i shouldnt moan but im sure im supposed to have a new care plan each year and trying to get her to do one for me is like getting blood out a stone she keeps forgetting , do you think this will be ok? anyway ive sent it off now . , i also sent them a copy of my med 3 form though they say dont send these to them send it job centre when i asked for my ESA85A it states clearly on it , that they put me in support group due to Evidence considered ESA50, med 3 , cosultant letter , gp letter etc

You can only send what you have, my wife has been interviewed multiple times for a new Care Plan over the last eight years, not one of them has resulted in a plan actually being produced.
Try not to worry too much, if you have completed the ESA50 in detail then you have done everything you can to ensure that your award is extended.
