If you are not able to make a call yourself then could someone make it on your behalf, you would need to be present but they should be able to handle the call on your behalf.
If no one can make the call for you then you need to write to the DWP to ask for a PIP1 to be sent. The address is;

I do have to warn you that the members who have gone down this route have had problems initiating their claim, one even had to involve their MP as the DWP did not respond within the four weeks allowed, so you must monitor it's progress carefully and chase the DWP sooner rather than later.
