Hi
Is any or all of this on record at your GP or consultant or other health professional?
You need to look at the sub categories and decide which one you think you come under.
Then you need to think why. Then write it all in the box and continue on a seperate piece of paper if neccessary and make sure it is clearly labelled and attached to the form.
Then you need to think about evidence. If you have anyone under the description I put at the beginning get them to confirm you get scared, hear voices or can get angry etc. Do you take any prescribed mdication to deal with this? If you haven't told anyone go to your GP immediately and tell them. They may prescribe something and write you a letter confirming it.
The incontinence is not relevant to this activity.

Hi Thanks for your quick reply. Yes i have it all on record with gp. I am under a mental health team . Have various consultants letters due to the incontinence.
But under the going out part on pip I'm not sure how to describe my illness. I rearly go out and if I do its always with someone

Hi
Can you get a meeting with someone from the mental health team to help you explain about going out?
They could also then write something to back you up.

Hi Karen I have similar conditions to you, I found being with the MHT helpful with my PIP application especially being on the recovery team / recovery plan they referred to this a lot in my application. So if you have anything like that I'd include it along with your bowel consultant's letter about your bowel condition. However I got enhanced rate care but not mobility. Good luck xx
