Still filling in the form, I have physical health problems but when it asks about going out in section 15 under mental health I kind of want to write something.
Simply because I do get worried and nervous if I have to go somewhere new, but it's my physical issues that cause me to worry, will I get a seat, will it be busy, will I get knocked over all those questions cause me to get anxious. Now I don't have any diagnosed anxiety or panic attacks but that doesn't mean to say I don't get worried.
So because I have no actually mental issue here should I just tick I'm fine and leave the box blank?
Should I tick I'm fine but then write something similar to the above in the box?
Or something else?

I'm having the same debate with myself regarding ability to carry out daily activities under mental health. The pain causes me to feel low and so some days I might not always get dressed
There's no harm in explaining the anxiety you feel going out, even if this is primarily due to physical causes, you lose nothing by doing so and you may score points if your issues are considered to be serious enough.
You can do this in other sections, but I would be circumspect in doing so, the more you stretch you symptoms to fit the less credibility you may have if you are not believed and this may spill over into the activities where you have a reasonable expectation to score points.
