ESA Capability for work questionnaire

I have been on ESA and in the support group for quite a few years. I have secondary progressive M.S. I have recived ESA50 04/15 in the post to complete, the form prior to this was ESA50 01/13.

Which guide should I be using?

I've looked at the guides but I'm still unsure. As this is a review do I use the date when my claim was originally made to find the necessary guide?

Hi a,

It doesn't matter that it is a review/reassessment, you should treat it as if it was your first assessment !

You should always use our most current Guides which are available from the links 2nd and 3rd down the page that Gordon provided the link for.

Sending Documents to the DWP or Maximus. (aka CHDA, aka HAAS.)

Don't assume that The Assessors who are now : The Centre for Health and Disability Assessments (CHDA) (aka HAAS) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15. will have any access to your previous documents/evidence, the likelihood is that they will not.

They may not even be aware that you are currently in The SG !

So, you should clarify how and why you still qualify for The SG !
