ESA Form was created by Lynne Christine Hollos
Hi,
The ESA50 form arrived on Friday last week, I am have with rhemotoid arthritis and have had this now for more than 35 years. My daughter comes every day, would it be ok for her to complete the form? as I find it difficult to hold and write with a pen,I have booked an appointment at my GP as well who I see regularly basis.
I honestly don't know where to start, I receive high DLA on both counts, and have it indefinetly, I haven't transferred onto PIP yet but I suppose that will be coming as well.
Kind Regards
Lynne

Hi,

Welcome to Benefits and Work Forum :)

If your forum name is also your real name, you may wish to change it to maintain some anonymity.

This can be done quite easily by following the instructions in this FAQ :

My full name is showing, how can I stop it?

You don't say whether this ESA50 is as a result of your first assessment for a fresh claim or due to a re-assessment of your existing ESA award !

Either way, you should treat it as if it was your first.

There is no problem with your Daughter filling in the ESA50 for you.

She does not have to write in 3rd person, unless she wishes to.

She can simply fill it in as you and you can sign the form if you agree with the contents, or she can fill it in and state in the form that she has filled it in on your behalf due to the restrictions that you suffer in using a pen !!

You could of course use The PDF Interactive ESA50, where the answers can be typed, edited and saved to your P.C. if that helps.

The best way to approach it is to go through the ESA50 question by question whilst referring to the corresponding section in our ESA Claims Guides.

Have a read through : This Post and the information from the blue links therein.

If you then have any further queries you can post them by replying to this topic.

You should Bookmark/Favourite this page in your Internet Browser now. That way you can return to this topic easily to pose any further queries that you may have by replying to the topic.

Hi,
Thank you for reply, my name is lyngold77 so I will be using that in any future posts.
It is a reassessment as I have had it when it changed from incapacity benefit a few years ago, then the esa50 form was completed by DART, it went through the system ok.
It makes me feel phsically sick when we look at the form, the questions to us feel very confusing, but we know like a lot of other people out there it has to be completed and returned.
My daughter keeps telling us to stop worrying, but we have joined the benefits at work as it makes it simpler to complete the form, giving us some very important information on how this works.
My husband and I are both 60, so people that live on their own it must be worse.
Thanks again for your advice on how to use the site, and will let you know yo the outcome.
Kind Regards

Hi,
Thank you for that, just thought I would keep people's updated, I contacted DART and they are coming to help my daughter and I fill the ESA Form in next week.
That is a relief for me and my daughter, I have read a lot on the site which is very very helpful.
I will keep everyone updated.
Thanks again and keep up the good work, if it wasn't for you people wouldn't know what to do.
Regards
Lyngold77
