Won't bore you with long drawn out problems. Been on disibility for years after accident at work. Went on esa support after successfull appeal of wrap. Waiting for change from dla high and middle to pip. Condition worse due to high blood pressure and degeneration of spine and hip. I.e spondylosis in neck and spine. Question is I only see doc once or twice a year cos told me no more operating would help. Will atos use my medical notes from esa or will I need further proof. My doctors surgery is group practice and my original doc has retired. I can hardly get further proof if atos received the initial details themselves from my initial success. All a bit confusing. Also worried about venue in hull for any medical. I can't walk far and use a cane all day. Have a mobility scooter but hardly go out on it Tbh. I've had adaptations to property. I.e. bath lift hand rails for loo and bed. Already have chairlift. Any help folks would be appreciated for when I receive the dreaded form

If your forum name is also your real name, you may wish to change it to maintain some anonymity.
This can be done quite easily by following the instructions in this FAQ :
My full name is showing, how can I stop it?
I would not rely on DWP or ATOS requesting and/or referring to any of your previous evidence !!
Nor would I rely on them contacting your G.P. or any other treating HCP as they seldom do even when requested to do so and are not legally obliged to do so !!
Our advise to members is to obtain their own evidence such as a letter of support from one's G.P, and/or any ,medical reports, etc.
If you are going to submit older evidence with your PIP 2 Form when it arrives through the post, it is fine to do so as long as you make it clear that your medical conditions and resulting limitations have not improved since the date of the evidence.
If your G.P. refuses to support you, you can make a request for your records and use extracts from that :
Requesting your medical records
Charging for medical records
You should be aware that the qualifying criteria for PIP differs greatly to that of DLA, so you will need to approach it very differently.
ATOS cannot compel you to make the return trip to/from an Assessment centre that would take 90 minutes or more either way using Public Transport
They are also supposed to take your limitations into account before deciding whether you should have a Home Assessment or an assessment at one of their Centres.

If you wish to request a Home Assessment, you should request this clearly in a letter with your PIP 2 Form.
A letter from your G.P. supporting the fact that you require a Home Assessment explaining why you require a Home Assessment would help considerably, and if you can gain one you should attach copy of this to your letter with The PIP 2 Form.
