PIP Reconsideration Advice

I was recently refused PIP, scoring 6 points for daily living and 4 for mobility. I have been advised to ask for a reconsideration, so called yesterday and the woman on the phone asked what I disagreed with, then said it could take up to 9 weeks for a decision.
Is that all I have to do? I was under the impression they would send me a form to fill in. I don't cope very well with speaking on the telephone, so was in a bit of a panic, and forgot everything I wanted to ask in my desperation to end the call!!
I received my initial decision on 12th February, but didn't think it was worth all the hassle of asking for a reconsideration until speaking to a disability advisor who said I should definitely do it , so I'm worried that if I'm supposed to fill out a form, it might be too late.

Welcome to Benefits and Work

There is no pro-forma to fill in to request a Mandatory Reconsideration (MR) of an adverse PIP Decision, but you must make the request in writing and post it to arrive back with DWP PIP no later than one month from the date of the adverse PIP Decision Notice letter.

You have one month from the date of the PIP Decision Notice letter to make a written request for a Mandatory Reconsideration (MR) of the PIP decision.

Monday to Friday, 8am to 6pm
To request a copy of The PA4 PIP Medical Report compiled by the PIP Assessor at your face to face and sent to The DWP PIP DM to be used when the DM came to their decision.
If you have any problems obtaining The PA4 with a verbal request, you can make a written request which should be done as a separate issue to any MR request :

Or you can make the request in the form of a letter, as long as you make it clear that you are making a formal SAR under The Data Protection Act 1998.
Address the envelope : FAO The Data Controller, at the Benefits Office dealing with your PIP.
Write DPA 1998 SAR in front, top, left corner of envelope.
Once received, they have 40 calendar days to comply.
You should not miss your one month deadline if The PA4 has not arrived.
You can make comment that you have requested this and will submit further evidence if applicable when you receive it.
You should be aware that when you request an MR, any current PIP Awards, Daily Living and/or Mobility, that you might have, can be increased, reduced or taken away altogether, as they will look at the whole PIP Awards afresh.
Obviously if you have no current PIP Award, there is no risk involved.

Once you have gone through The MR Process, if you are still unsuccessful, you have one month from the date of The MR Decision Notice Letter to appeal directly to The Tribunal Service. (TS)
You should do this using copy of The MR Notice letter and the : SSCS 1 Form (Download Link)

You should concentrate on reiterating where and why you should have scored the extra points with respect to the applicable PIP Activities and the appropriate scoring PIP Descriptor for that Activity, whilst cross-referencing to any medical evidence that you have, or may have already provided which indicates that you should have.

Also, I meant to ask this in my first post, but in the report from the assessment, the health practitioner has written a short bit about how my illness affects me in each descriptor section, but then further on in the report, when it comes to giving points, the reasons not to award points seem to contradict what she has written at the beginning! I originally thought that what she'd written at the beginning was just what I had told her, and the latter sections were her assessment, but now I'm not sure. Is it the HP or the decision maker who fills in those bits?

The PIP Medical Report compiled by The PIP Assessor at your F2F is a PA4 Medical Report there is example of one from the link "PIP Sample medical report Our own sample version of a PIP PA4 medical report form" 4th link down : This Page
This includes recommendations made by The PIP Assessor on which Activities/Descriptors you should score points.
Then there is the PIP Case Manager's (DM'S) Reasoning for making their decision.
The PIP DM can agree or disagree with The PIP Assessor's recommendations made in The PA4.
