Cruella wrote: My GP is phoning tomorrow, and I need her to do a letter like she did fr my PIP , only this time for ESA, saying I cannot get to a medical assessment centre. I am thinking she could use the same letter only change the date and the address. I have not worked out what she needs to put as the address on the letter, whether it should be Maximus c/o DWP? And what address? DWP address or Maximus address?
It will be Maximus as the will be the ones that will be making the decision on a home assessment.

Cruella wrote: would it be the norm to send in GPs letter saying you cannot attend a medical assessment centre actually with the ESA50 form?
You can send it with the form, but don;'t be surprised if they miss it.

2)My understanding is that virtually everyone has to attend this first work-focused interview (because it is not decided whether you will be in the support or WRAG at this stage). Am I correct, if so what happens if like me you cannot attend a job centre due to health conditions for this first work-focused interview? Would they do it over the phone?

It is something you would have to discuss with the JC+ advisor, it is possible for them to hold the interview over the phone, to defer it or even to cancel it. They are required to take your limitations into account when requiring you to attend.
