If the information is relevant to your conditions as they are now, then you need to think about including it, it's easier for a Decision Maker to understand how a broken leg would affect a claimant, but a lot harder for them to understand how mental health issues do.

It can take up to 40 days though , can t it? So does that mean a form may have to be sent with the evidence to follow on later

IF it took 40 days then you may need to send the information after returning the form, but as I said, most requests are answered in no more than a week or two.

Hi again,
If I decide not to go for ESA (cos prob not eligible) but because I am definitely going to claim PIP, should I use my fit/sick note as evidence for PIP ( I know you don t need a fit/sick note for PIP) Is it worth continuing to obtain fit/sick notes simply to use as evidence for PIP ? (I am not working)

Hi C,
It's up to you, but fit notes are issued with regards to your capability for work, PIP has nothing to do with your capability for work or lack of it.
In fact, you can work full time and still be awarded PIP if you qualify !

Next question lol
I have just spoken to my surgery who seem to think if I write in asking for copies of my medical notes that when they are ready I have to go in to pick them up , or someone else possibly can pick them up. But I am unable to go to the surgery myself and do not want anyone else picking them up. Any thoughts on this please
