esa50 question 1 was created by misty blue

Filling in form for reassessment for my wife
She cannot self propel wheelchair at all
She now has an attendant wheelchair after o.t reassessment

There doesn't seem to be a tick box for her situation in the first question in movie g around
Any ideas how to deal with this

If the tick boxes do not cover your wife's limitations adequately then you can mark them "see below" and answer in the comments box.

Even though your wife has been assigned an attended wheelchair, you must still explain why she cannot self-propel a manual wheelchair.
