Has anyone heard of some type of medical care plan given by GP listing all diagnosis, medications, and limitations for patients? One person is adamant that her GP/nurse filled in a form listing all her diagnosis, medications, physical limitations etc which she sent along with her PIP form. She would't give more info, ughh
she carries this form with her at all times in case she ends up in A&E and gives it to other public sector workers involved with her care. Unfortunately she does not like to share info, and refuses to part with ,more info.
