GPs Medical patients care plan form??? was created by chafur47
Has anyone heard of some type of medical care plan
given by GP listing all diagnosis, medications, and limitations for patients?
One person is adamant that her GP/nurse filled in a form listing all her
diagnosis, medications, physical limitations etc which she sent along
with her PIP form. She would't give more info, ughh

she carries this form with her at all times in case she ends up in A&E
and gives it to other public sector workers involved with her care.
Unfortunately she does not like to share info,
and refuses to part with ,more info. :evil: :P

Hi c47,

I have heard of some Consultants setting up medical care plans for a patients G.P. to follow.

There are also : Care Plans set up by Social Services .

Maybe, this is something that their G.P. has provided for them off their own bat ?
