does maximus have email address?

I received the dreaded reassessment form just before easter... trying hard to concentrate long enough even to do a bit a day. I saved the online version and Have been filling it in and have just noticed it is esa 02 15 Will it be okay to send this one back or do I need to find the 04 15 one and start again? brain fog and sky-high anxiety not helping me think this through and I am sorry to be a pest but so grateful if you could advise me on this.

You should fill it in whilst referring to the guides on this page, 2nd & 3rd links down :

You should treat this re-assessment as if it was your first assessment and not assume that The Assessor will have access to any of your previous evidence. They may not even know that you are currently in The SG, so you should make it clear that you are.

It is therefore important that you reiterate how and why you still qualify for The SG, in that the limitations that you suffer as a result of your medical conditions have not changed nor diminished to such an extent that you no longer qualify for The SG.

Nor have you learned to deal with or adapt to your limitations since your last assessment.

If you are aware how and why you were placed into The SG last time, you could cite that also.

If you want to obtain fresh evidence such as a letter of support from your G.P., obtain it yourself and post it with the ESA50.

Do not rely on Maximus/DWP contacting your G.P. , even if you request that they do so.

They seldom do and are not legally obliged to do so.

Just to make you aware that : The Centre for Health and Disability Assessments (CHDA) (aka HAAS) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15 now carry out ESA Assessments/Re-Assessments.

There have not been any changes to ESA Legislation since January 2013, however you should always use our most current Guides.

There have been some changes to The ESA50 itself.

my last esa 50 was atos assessed and i had an email so was able to check if they had received it. i had to send it 3 times by special delivery before they confirmed receipt. in high anxiety trying to fill in re assessment form again, brain bouncing everywhere and like trying to catch water in a sieve trying to hang onto a thought so really struggling. sorry i digress. Do you know if maximus have email address contact so I can do the same and know for sure when they recieve it? tried to search for this answer so sorry if its elsewhere... fog really thick

I had reason to email Maximus just a few weeks ago and got a very prompt and helpful reply.

I am so sorry bro I had a message saying i had used a wrong form and needed to resubmit and didn't bother then thought about maximus email. I do apologise I should have checked. topic now book marked.
thank you for all info. I have found.. oh okay husband found for me the right form to fill in online 04/15, so back on track/
