Assessment Reports was created by poohbea1
Hi, I live in west Wales and I have received my PIP result after my home assessment by Capita, which was less scary than I ever thought possible. The nurse was lovely and put me at ease straight away, having noticed my crutches, walker and some gadgets as she walked through the hall into my lounge to meet me. I have been awarded Enhanced Care and Enhanced Mobility, having previously been in receipt of first level care and high rate mobility of a DLA Indefinite Award. My conditions have worsened over the years but didn't apply for reassessment as the process is so stressful. On the paperwork I have received it has stated how many points and what sections they were awarded for. Is this the same as an assessment report or are the two entirely different? If different then I will want to apply for the report to make sure I am fully informed when I am reassessed in 2022. I am guessing that the papers or computer information will have been disposed of by then. Who do I apply to for this information please?. Without Benefits and Work both my husband and I would have been completely up a creek and without a paddle. Thank you as you were also helpful with ESA as well.

Hi E and Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

Welcome to Benefits and Work

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

My full name is showing, how can I stop it?

It's great to hear that you got the PIP award that you deserve! Thanks for letting us know and for your kind words about B+W :)

The decision letter is not the same as the assessor's report, which should go into more detail. In addition, it is possible (although not common) for the decision maker to have given an award contrary to the assessor's recommendation.

You can obtain a copy of the report by contacting DWP and asking for it.Or, if you prefer, you can submit a Subject Access Request for all the paperwork associated with your claim. DWP would have 40 days to comply with this.

Requesting a Subject Access Request (SAR) :

SAR Form Download

From this page :

DWP Data Protection Policy

Or you can make the request in the form of a letter, as long as you make it clear that you are making a formal SAR under The Data Protection Act 1998.

Address the envelope : FAO The Data Controller, at the Benefits Office dealing with your PIP.

Write DPA 1998 SAR in front, top, left corner of envelope.

Once received, they have 40 calendar days to comply.

Sending Documents to the DWP.
