information on how to fill in pip forms

Please can you send me some information on how to fill in the PIP forms, I am 66 years old, suffer from Angina, COPD, High blood pressure, Type 2 diabetic. At the moment I receive the higher mobility and care components but am afraid I might lose my benefits and will lose my car and benefits and any outside help.
