PIP review was created by Jeff OBrien
I am writing this on behalf of my husband who was awarded the Daily Living component of PIP from 25 November 2013 to 31 March 2017 (both dates included) due to his acute anxiety disorder. He has become extremely upset ,anxious and confused as he received a letter 2 weeks ago supporting the decision as stated above in the DWP letter(09.06.2014). A second letter arrived last week stating that they are looking again at his PIP and he has to complete an information booklet and he may need a consultation. If he doesn't return it by 02.05.2016 his claim to PIP may be disallowed. As I am sure you will know this has caused him considerable upset. Should I contact DWP and query the 2 letters or do we have to follow the request to submit the PIP form. I would really appreciate your guidance and be able to reassure my husband. Many thanks. Welcome to the forum, you might want to have a look at the following FAQ which explain where everything is

Welcome to Benefits and Work

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

My full name is showing, how can I stop it?

I'm afraid nearly all PIP awards are being re-assessed year in advance of the current award ending, the letter you received two weeks ago is simply the annual entitlement letter .

This is a full reassessment of your husband's claim and he needs to complete the new PIP2 in detail and provide evidence in support, even if this was previously submitted.

I would recommend that you have a look at our PIP Claim guide on the following link which will help you complete the form.
