Hi, I am in the support group for ESA at present. I've just gone through a horrible DLA to PIP transfer and got enhanced rate at both care and mobility and now have this form. When I was first transfered from IB to ESA I didn't know that I could ask for a copy of the report. As this was 3 years ago do you think I could still ask for a copy, though whether it would arrive before I have to submit the form is another matter.
I also wanted to check which is your latest guidance booklet on completing the form?

The latest guides for ESA are at the top of the ESA section.

You can issue a Subject Access Request for the old report. It may no longer be available but you lose nothing by asking.
