PIP Mandatory Reconsideration Letter

Hi, can someone help me. I am a subscriber and need to write a Mandatory Reconsideration Letter for PIP - I know there is some downloadable information on how to write the letter, wording to use, things to include - but I can't find it here on the website. Can someone point me in the right direction? Thank you.
