I need to report a C of C to DLA office regarding a change in my mobility. My mobility has actually improved over the last couple of months. When I call regarding this, how much information will they require? Or will they just send me a form to claim PIP? thanks.

They'll take basic information of the change over the phone, you won't be speaking to anyone with authority to make a Decision, so they will collect it and pass it back to those who do, but the most likely result is that you will be invited to claim PIP.
