Forgot to photo copy PIP form was created by JLP
Hi,
I can't believe what I've done. I filled in mŷ husbands conversion from DLA to PIP form. I've copied all the medical evidence. But I've just realised that I have forgotten to photo copy the claim form. I can't believe how stupid that was. Any one got any ideas how I could get a copy of it? from DWP.?
Will it be a strange request from a claimant and look like I'm up to no good!
Worryingly my husband has Altziemers and he won't remember what we wrote.
Any help would be gratefully appreciated.

Give the DWP office dealing with his claim a call, they should be able to send you a copy, failing that, issue a Subject Access Request.
