Health Assessment questionnaire NOT received.

The centre for health and disability assessments wrote to me on 27 June 2016 advising me that on the 3 June 2016 they allegedly sent me a questionnaire to fill out and return to them by the 6 July 2016. However I did not receive such mail and contacted them on 30 June 16 when I received their letter to inform them that I haven't received it. Debbie from ESA in Manchester confirmed that she would request that a new questionaire form would be sent out to me and that I am to fill it out as soon as I can whilst enclosing a letter along with it to explain why the questionaire is late.

Today the post has been and gone and I haven't received the questionaire still?

I am now concerned that they will stop my claim and they are extremely difficult if not impossible to get hold of via telephone.

Any advice would be very much appreciated thank you.

You are right to be concerned as there is little leeway in the date for returning the form. It might be worth informing DWP and HAAS that the form has not arrived but also downloading the PDF version and completing that.

I have until the 1st August 16 to complete the form. I have been feeling at my lowest recently and now my son has finished for the summer term but I know I have left this late! My memory is terrible. I wish I was in the support group so that they could give me more support as i am really struggling to cope with life right now and all I have going on.

Can anyone assist me please with filling out the Health Questionaire?

I have the health Questionaire but my brain is unable to put my thoughts into writing. I am unable to concentrate and my memory really is awful. I don't even understand how to use this site correctly to get full use of it.
