Hello

I am under a great deal of stress, pain & panic trying to complete both an PIP2VO & ESA50.

My anxiety is causing confusion and I'm struggling to start a fresh with the guides. Could someone please bullet point me what I need to download for each form - I will brief detail my health down below. Any advice asap would be greatly received X I have a DWP home visit to help complete my form but am afraid they won't allow me time to include the important points if I don't prepare as much as I can. I don't know if it's because I only have mobile access but I just can't get my head around it at all.

My grandchildren have chicken pox so my daughter is staying with them at their Nannys tonight so she can watch them in the morning and she can get here for my appointment.

My DLA has been stopped after 4 yrs as pip say they never received my form (first time ever posted with no proof!!) and Esa has been cut substantially to the point I am 4 weeks away from being served eviction papers!

Esa won't provide home medical since capita have taken over. The home medical team have been very supportive and helping my daughter to fight this since November.

I have mental health problems - diagnosed PTSD, Panic disorder, agraphobia, Generalised Anxiety disorder, Night Terrors and in off depression that prevent me from going out without support and only to my GP which has been twice since October. I also have physical conditions since an accident constant severe pain in my back, hip, neck, severe migraines weekly, nerve damage due to b12 deficiency which causes numb hands, feet, arms and pins & needles in my head and face, CFS, Fibromyalgia and last but not least the one that is the most problematic in term of going out (bowel incontience due to surgery botch) that I have in writing 'is incurable and lifelong so I am therefore afraid I will be unable to conduct any further investigations or procedures and am handing you back in to your Gps care' - because this is dated 2012 and was in the hands of atos I have been informed that capita want all new medical records?? Is this correct and legal or can someone advise me of any law against using documents by a certain date?? I was advised to medically retire by a specialist in 2012 aged 41! Worked all my life, ran my own company and I am now begging just to be able to keep a roof over my head. I personally would rather not wake up tomorrow and if it wasn't for my daughter and grandchild I doubt I would.

The first thing you need to think about is how you are going to manage these two forms? Although at first glance there may appear to be significant overlaps between what ESA and PIP look at, the reality is that the criteria are usually quite different so you might want to think about completing the forms one after the other so you do not confuse what you are being assessed against and document the wrong explanations.

Try and break the work down into manageable amounts, for example prioritise the Activities that you would most expect to score points for over those that you don't, try and start with sentences and then build them up into paragraphs.

The guides for ESA are at the top of the following link, there are two, the physical one covers question 1-10 on the ESA50 and the one for mental health is for questions 11-17.

We can explain the criteria to you but only you can document how your conditions affect your ability to complete each of the activities.

It's up to you what evidence you submit, but those assessing you will pay less attention to older reports than they will to new ones, so you need to show that those reports are still relevant to how you are today. If you can show that your conditions are not ones likely to improve or indeed that they can only get worse then this should bring your old reports back into relevance.

Have you spoken to your GP? They may be able to provide you with up to date information, just be aware than many GPs expect to be contacted by those assessing you, this is very unlikely to happen and they need to understand that not helping you at this stage is likely to have a detrimental effect on your health.

Well the form filling experience was not as expected. I had to leave my mum alone (my child emergency) - with the front door open so the DWP home visit team woman could see my mum in her recliner and mum could sleep till she came (used to work at supporting pensioners to complete forms but now are focused on what they call 'vulnerables' Mum was informed on Thursday) X Not an ideal situation but Mum insisted through fear further delays were going to cause eviction. Mum states that the DWP woman arrived pulled out a pip form and went through her day basically. I'm quite concerned that the details as stated in your documents that I used on the first form which they state they never received may not be included though I did keep saying To mum that I had put some notes on her phone to sections of your documents I had, bullet points mainly 1. Use the word 'reliably' when describing things you can occasionally do. "I can occasionally though not at all reliably make it to the toilet alone using my bed frame to sit up and 2 walking sticks as it just 5 steps away from my bed to sitting on the toilet"

Mum says she said 'not reliably' repeatedly but fell asleep twice during the appointment and she doesn't remember the word 'reliably' being heard once when it was read back to her at the end.

I do feel it went as well as can be expected considering the circumstances and I called and requested copies of the form and any thing else now sent to the pip decision makers. The supporting evidence is going to be the biggest problem with it all dated 2012/13 though stating "incurable condition that is very likey to deteriorate over time" so I'm going to have to try to get maybe a phone appointment with her 'useless, unhelpful, unknowledgeable, uncaring, socially dilisuioned GP who though it is documented that my mum is bedbound approx 85-90% of the time and his appointment system and home visit receptionist only ever gets him to call not visit now means quoted verbatim 'has had 2 needs for appointments in the last 18 months so will assume that the treatment plan is working well and Miss C####y is managing well at this time' - hmmm my sister has a list of 12 requests for home visits, and 47 surgery appointment call dates when no appointment was available and because the receptionist decides what is a emergency and has never been accepted a call to 111 has resulted in hospitalisation twice, paramedic/emergency dr home visit to reduce blood loss through injections on 3 occasions, and the mental health crisis team being involved half a dozen times - this information never made it on to the form showing medical attention WAS required and given though not though her ineffective GP.

They are calling this a reconsideration as her first form wasn't received so pip was declined - now this form is in, can I quickly send a few letters of support and a real list of medical treatment mum has reviewed and also add that going to the dr has not been done through Mum's anxiety and agraphobia and show that many requests of medical support were made (I have detail phone records) to the GP but not given.?? Or should I hold this back in case of an appeal?

Sorry to be a pain - stress rambling!!

Q1. As they are calling this a reconsideration as her first form wasn't received so pip was declined - now this form is in, should I quickly send a few letters of support and a real list of medical treatment mum has received and also add that going to the dr has not been done through Mum's anxiety and agraphobia (not good health as he has stated) and show that many requests of medical support were made (I have detailed phone records, some recorded) to the GP but not given.?? Or should I hold this back in case of an appeal?

Q2. Is there currently any loan or grant services available to people on benefits as a) my mum is getting to the point of possible eviction papers being served.
b) She is in desperate need of some kind of medical grade mattress due to a couple of her conditions I have noticed that though cleaned and dried by myself I can see what looks like a mould growing and mum has recently been coughing constantly so she is now sleeping in her recliner chair I which is causes more pain for her.
c) my own health is becoming a problem through lack of sleep/rest having a 4 month old and 3 yr old and though i want to care for my mum I just can't do it all and even if I could get someone to help with her washing, keeping her room cleaned, collecting prescriptions etc

I know we are just one of many people in a desperate situation and I pray a positive outcome for all am feeling at a loss of want I should be doing X
