In the walking example you gave what if the person could physically walk but is there was a road they would not bother to stop and look before they crossed it to see if a car was coming thereby putting their life in danger would that count?
Mental illness is so subjective, I feel these questions are not that straight forward to answer.

It's just an example to indicate the difference between the two, but to answer your question, there are two PIP activities that are used for the Mobility award, the Moving Around activity considers the claimants physical ability to walk, the Going Out activity looks at the problems they would have planning and following a route due to mental health, cognitive or sensory issues, from what you describe your sister would certainly expect to score points in the latter.
You may need to start from scratch, the criteria for PIP are very specific and very different from DLA and the PIP2 does a poor job of actually explaining what criteria a claimant is actually being assessed against.

It's too early to say, but one option is to make a new claim for PIP but my meaning was that when you look at the PIP Claim guide you may realise that you have not explained your sister's problems in a way that matches the PIP criteria and for the MR you might have to go back to scratch and explain them again.

I have had a look at the PIP form. I do understand it better now however now running through it does appear that the assessor chose to completely ignore my response. For example in the food section the claimant needs prompting to cook simple meals and we supervise in case of an accident. The assessor chose to go with my sisters answer based on a example she made a meal 5 months ago leaving out the bit we supervised.
The problem is that my sister as part of her condition lies to cover it up and gets very defensive. Stubbornness and violent tempers are a big part of her symptom. Even if we did this assessment again the problem is my sister will not give the full picture and I'm worried the assessor will ignore what I say. Only the doctors know her condition as they have seen it first hand over the course of 6-7 years. You need more than 45 minutes to assess her.
She does not have the mental aptitude to be 'prepared' for an assessment for PIP nor would we want to. Is there a way to do this by simple obtaining the doctors verdict?

Another point is on definitions. What is the definition of a simple meal? Is making a piece of toast and heating a tin of soup a simple meal? She can do this however does need prompting. Her medication and condition have made her slow and tired most of the time.
