I am Expecting the Dreaded PIP Letter and Form any Day now as I am on DLA (Aged 62 & 8 months). I may have missed where to get one but I am after a Blank Pip form so I can Pre Complete it and Play around on it. I have Downloaded the New Guide from B & W but cannot see a Link for a Blank PIP form. Any directions to one would be appreciated.

You can download an example PIP2 on the following link, but you must use the PIP2 sent to you when you make a claim, each page will contain a barcode unique to you, a failure to use the issued form can result in your claim being rejected.
