Hi,
currently in ESA Support group. Just got my reassessment form. have multiple chronic illnesses.
Downloaded the form from gov.uk but it has a different code at the bottom right of front page and rear page.

Is this important as I cant see any differences between the two form contents.

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

My full name is showing, how can I stop it?

It should not be a problem the only changes to the ESA50 in recent years have been cosmetic ones.
