I am not sure whether this has allready been asked, but does anyone know where you send a subject access request for information after an assessment,I am unsure of which department to send to. Also where to send an MR request. Also I have allready sent a recording request for my ESA assessment,in writing, but in the correspondence it says telephone if you need to record. Is there an email for Maximus as I would prefer it in writing.They did not acknowledge my request attached to my ESA50.
If it is in regard a recent assessment then you do not to use a SAR, you can just contact the DWP office that dealt with your claim and ask for the reports.
If the reports are older then send the SAR to the office that dealt with your claim.
An MR request should be sent to the office that dealt with your claim within one month of the date of the Decision.
