ESA 50 was created by tigercub1
Hi, I have recently sent off my esa 50 (re-assessment) and neglected to copy all of it as I was v.stressed trying to complete it and post by deadline. What are my chances of obtaining a copy from dwp? I also need to rearrange my assessment appt. (end of this Dec.) in order that my partner can attend with me. Any advice appreciated

My full name is showing, how can I stop it?
You can contact DWP and ask for a copy of your ESA50 form - but I wouldn't expect them to release it until your claim has been decided.
It is Maximus, not ESA, who arrange the face2face assessments - I won't call it a 'medical' as it will be unlike any other medical you might have undergone! Our ESA guide explains what to expect on the day.
ESA claims and appeals
The claimant is allowed to change the appointment only once without penalty so make sure your partner has his diary to hand when you phone up to do this.

This will allow you to return with further questions or comments about your PIP without having to start anew topic each time. Hi, following on from my other posts I have been awarded same as I already get ESA (wrag)but received no explanation in the letter(23 Jan). I have asked for a mandatory reconsideration and a copy of all information they used in making decision in writing as you suggest. I also now have a letter from my g.p with medical history in support of exempting me under reg.35 .
I have heard nothing from DWP yet. My question is should I phone them or send in my g.p's letter or both.
