Is it worth sending in a copy of my stretches, in with my additional information as evidence?.

Do I need to include my physiotherapists HPC number?

If you can link your stretches to one of the descriptors (managing therapy perhaps?) then by all means send it in.

I don't see any reason why you should have to include the physio's registration number.
