I noticed that the above sample form has been updated for October 2016. It no longer mentions the importance of being able to do various activities 'safely, to an acceptable standard, as often as you need to, and in a reasonable time'. I am unable to find the Oct 16 information booklet that would accompany this form. Does anyone know if the information booklet stipulates this still, or has this wording been omitted completely now?
