Support Letter

I'm afraid there is no template for this as every letter will be unique.

They need to provide their name and address, how they know you and how often they see you and lastly they should make some sort of statement as to the information being true and sign the letter.

As to what to include, they need to explain the difficulties they have seen you have with completing each of the PIP activities, how those difficulties manifest themselves and when.
