PIP Assessment

Hello all,
I haven't filled in a how my disability affects me form but I am having an assessment next week. I was wondering if this has happened to anyone else and is this new procedure? I'm very confused by all of this. How on earth can I be assessed without the form, they will have nothing to assess me on surely. Any advice would be much appreciated.

Is this a new claim or reassessment?
