Hi,
I am wondering if I am filling in the correct form here, I have a PIP1003 form to complete for changing from DLA over to PIP. Is this the form that you provide the new 2017 guide for as my questions dont seen to be the same as in your guide.
Can use please provide a link for me as I struggling to complete this form which needs to be sent asap.

The 1003 form is the PIP2, the latest PIP Claim guide covers this.

I am completing a PIP application for my son, his DLA is at an end. I have been sent form PIP 1003 but it has no descriptors/tick boxes to complete. Is this the right form?

Sorry but from your description I cannot tell you, here is a link to a PIP 1003 form to see if your form looks similar, you may want to check with DWP that you have been sent the correct form.
