My husband is on high care and high mob DLA.
just had PIP forms. He has spinal stenosis and is always in pain controlled by fentanyl patches and morphine sulphate tablets which make him drowsy.
He can manage the pain as long as he does nothing so I help him wash dress and cook all the meals. once he tries to do any of these tasks the pain increases and he has to take tablets which make him drowsy. when he fills in the form for these activities is it in order to say that he needs help with every aspect of the tasks of dressing washing and cooking because of the increased pain they cause

There are a couple of aspects to this.

Yes you should explain assistance that he requires in order to complete the PIP activities.

He is not required to complete them pain free, but any pain that limits his ability to do so should be considered.

Have a look at the use of "reliably" and "on the majority of days" in the PIP Claim guide, as these may offer you other ways to explain his restrictions.
